If you would like to use your Run For The Arts funds for the services of an artist or arts organization that does not offer programming through Young Audiences, please fill out and submit the following form along with an invoice from the artist or organization:

If you are contracting with an individual artist (rather than an organization), that artist will need also need to submit a Statement of Fiscal Procedures and an IRS W-9.
